For your appointment, please bring a thorough list of ALL medications your are currently taking, including dosage information -or- bring all your medication bottles. Also, please remember to bring any self-monitoring blood pressure records and your blood pressure machine. This way we can check to be sure it is calibrated accurately. And please be sure to bring a referral if required by your insurance carrier.
